All eligible international students are automatically covered by/enrolled in the Conestoga International Health Insurance Plan. Please check your tuition statement to ensure that you are covered.
Your coverage card will arrive by email by Sept. 15th. If you have not received it by Sept. 15th, or if you have lost it, please click here to request a new one. You must show it to medical staff to receive care and be eligible for reimbursement.
Yes. You can request a new card here after Sept. 15, 2017
Yes. You can enrol your family (spouse and/or dependent children) in the Plan by paying an additional fee, over and above your own Plan fee. Common law and same-sex couples are eligible. Dependants must be enrolled for the same length of coverage as the student. Click here to add dependants now.
There are 3 ways to submit a claim.

Paramedical and Eye exam claims can be submitted in one of three ways:
- By Mail
- OR Online submission through our secure system (you will be redirected to Sun Life’s online claim form submission web page)
- OR MySunlife Portal & App. https://www.sunnet.sunlife.com/signin/mysunlife/home.wca
Call 1 844 418-0511 and dial 1 to speak with a Sun Life representative about the reason for your claim’s rejection, and possibilities for resubmission. Make sure to have your ID and Policy numbers handy.
All claims must be received by Sun Life Assurance Company of Canada no later than 180 days after the end of your coverage.

Upon visiting the clinic, show the staff your coverage card to have your invoice sent directly to Sun Life.
